Community Living Support
Procare provides community living services with bespoke person centred packages for individuals who have complex health needs combined with physical and/or learning disabilities, autism and challenging behaviour. This includes a care plan, statement of purpose and service user guide detailing their needs and wishes. We support individuals to enable them to live in their own home accessing meaningful participation within the community with up to 24 hour support 365 days a year. We provide a 24 hour on call service which is accessed by calling the office telephone number.
Through the use of the active support model staff teams will encourage service users to participate in all daily living activities. Data collection allows us to monitor and adapt our support plans which, together with person centred planning, enables us to formulate effective individual plans and work towards meaningful change for the individual.
As a theraputic behavioural service we view challenging behaviour as communicative and functional. In situations of need, people may simply behave automatically in ways which have been successful in the past. We therefore take a structured, evidence based and data driven approach to supporting individuals who present with challenging behaviours and we would therefore advocate baseline data being collected so as to inform any interventions.
Given that ProCare has a specialism in behavioural assessments and interventions, the Management team are skilled in reinforcing any programmes or guidance given by our behavioural team and Professionals. Indeed, with regard to daily living skills, many of the staff who currently work for ProCare are practiced in task analysis and the discrete teaching of individual skills. This ensures that as a service we have the infrastructure and relevant skills in place to support service users in achieving their full potential.
To best meet the needs of our service users, we firmly believe in imaginative flexibility - at an individual and corporate level. Wherever possible, we work in close partnership with others, integrating our services with local resources. Using an outcomes approach to service delivery not only helps us to enhance the services we offer, but it demonstrates to Local Authorities that they are funding a service that really does make a difference to individuals. What's more, we're constantly looking for ways to improve all our services.
Procare Wales have two residential care homes. Plas Lorna is in Rhuddlan and has been extensively re-built, modernised and refurbished from its former status as a large nursing home. It is a registered Care Home under the Care & Social Services Inspectorate for Wales (CSSIW) and is registered for individuals with complex needs between the ages of 16 and 64 and can be for both men and women. It is an eight bedded unit with all en suite individual bedrooms and spacious communal areas. It is also surrounded by an enclosed and safe garden which borders farmland. Attached physically to this care home but completely separate from it, are four self contained single occupancy apartments with their own unique entrances.
Arnold House is based in Llanddulas and is a five bedded unit with two attached but separate single occupancy apartments with their own entrances. The purpose of the apartments is to provide a less intensive time limited service for individuals prior to their move on to living in the community.
